Your Privacy Matters to Us

This Privacy Policy describes the types of information Vigilias LLC or Freestate Healthcare LLC, ("Freestate," "we," "our," or "us") may collect from you or that you may provide when you use the FreeState Healthcare HCP software application (the "App") or access our website (collectively our "Services"). We have provided this Privacy Policy to help you understand:

  • how we collect information that identifies, reasonably relates to, describes, is reasonably capable of being associated with; or could reasonably be linked, directly or indirectly, with a discrete individual ("Personal Information")
  • how we use, disclose, share, and retain your Personal Information, and
  • how we protect your privacy.

Please read this Privacy Policy carefully to understand our policies and practices regarding your information and how we will treat it. If you do not agree with our policies and practices, your choice is not to use our App or our other Services. By accessing and using the App and other Services, you acknowledge that you have read this Privacy Policy.

Personal Information We Collect from You

We get Personal Information directly from you when you use our App and our Services. Some examples of the Personal Information we may collect from you, usually by you providing it directly to us to use the Website or our Services, include:

  • First and last name
  • Email address
  • Phone number
  • Address
  • NPI Number
  • License Number(s)
  • DEA Number(s)
  • Username and password for the App
  • Employment History
  • Educational History

Please note that if you decide not to provide certain information to us, it may delay or prevent us from providing products and/or our Services to you.

Collection of Other Information

In addition to collection Personal Information, we may also record interactions that occur through the App, including:

  • Interactions with patients by video or text
  • Phone calls made through the App
  • Clinical information placed in the App
  • Billing Information
  • Other information related to your clinical work through the App

How We Use Your Personal Information

We use your Personal Information to provide you with the Services. We also use the Personal Information to verify the information you have provided to us to ensure that you are eligible and qualified to offer patient services through the App. Finally, we use your Personal Information and other information collected through the Services to comply with requests from law enforcement, regulatory agencies and health organizations in order to protect patients.

With whom do we share your Personal Informatio

In order to conduct our business and to better serve you, we may disclose Personal Information we collect or you provide, as described in this Privacy Policy, to:

  • Third Parties for Credentialing. Freestate may share Personal Information with third parties in order to verify any information you provide concerning your qualifications, education, background and skill. Freestate will only share Personal Information with each relevant third party that is necessary to verify the provided information.
  • Law Enforcement, Regulators and Other Parties for Legal Reasons. Freestate may also share Personal Information with governmental or other public authorities (including, but not limited to, workersÂ’ compensation boards, courts, law enforcement, tax authorities and criminal investigations agencies); and third-party civil legal process participants and their accountants, auditors, lawyers and other advisors and representatives as we believe to be necessary or appropriate: (a) to comply with any law applicable to us, including laws outside your country of residence; (b) to comply with legal process; (c) to respond to requests from public and government authorities including public and government authorities outside your country of residence; (d) to protect the rights, privacy, property, interests or safety of our company or our affiliates, customers, business partners, suppliers, employees or the general public; (e) to enforce our Terms of Service and other agreements; (f) to protect our operations or those of any of our group companies; (g) to protect our rights, privacy, safety or property, and/or that of Freestate, our group companies, our customers or others; (h) to respond to an emergency; and (i) as otherwise permitted by applicable law
  • Other Third Parties. Unless prohibited by applicable law, we may share, transfer, or delete your Personal Information if we are involved in a merger, sale, acquisition, divestiture, restructuring, reorganization, dissolution, bankruptcy or other change of ownership or control by Freestate or any affiliated company (in each case, whether in whole or in part). If required by applicable law, any such sharing of Personal Information will be reported to regulators in accordance with the applicable law

We do not sell Personal Information for monetary value.

Confidentiality and Security of Personal Informatio

Freestate is committed to ensuring that the data we use and process is secured while in our care and is used responsibly. We continuously seek ways to protect and ensure appropriate use of information related to consumers while promoting policies within the industry that do the same.

To these ends, we developed and employed controls that consist of policies, procedures, organizational structures, and technology to ensure consistent data security and to protect your personal information against unauthorized access, loss, misuse or alteration.

Your California Privacy Rights

We realize that you may object to the use or selling and sharing your personal information. If you are a California resident, California law provides you with some specific rights. We respect your right to privacy and intend to honor your choices. As may be required by law, you have the right to:

  • Know the categories of personal information collected
  • Know the categories of sources the personal data was collected from
  • Know the purpose for collecting, selling, or sharing the personal information
  • Know the categories of third parties the personal information is shared with
  • Know the specific pieces of personal information that is collected
  • Request that your personal information be deleted
  • Request that your personal information is not sold or shared
  • Access the personal information we have collected about you

Please note that these rights may be limited by applicable law. For example, there are various reasons where applicable law allows Freestate to retain personal information about you even if you have requested deletion.

Requests Relating to Your California Privacy Rights

Access to Specific Personal Information

You may request that Freestate provide you with the specific pieces of personal information we have collected or sold to third parties within the last 12 months. In order to make this request, contact Freestate, via email at info@freestatehealthcare.com, or call 800-924-8140, extension 1 and speak to a customer service representative or leave a message. Please note that we need sufficient personal information about you in order to provide personal information, which only relates to you. If we cannot verify your identity, or determine which personal information relates to you, we may not be able to comply with your request. If we cannot comply with your request, we will let you know within 10 days of receipt of your request.

Delete Request

You may request that your personal information be deleted from our database. Contact Freestate, via email at info@freestatehealthcare.com, or call 800-924-8140, extension 1 and speak to a customer service representative or leave a message. Your request will remove your name and address from our databases. This will result in you losing access to the App.

Please note that we need sufficient personal information about you in order to provide personal information, which only relates to you. If we cannot verify your identity, or determine which personal information relates to you, we may not be able to comply with your request. If we cannot comply with your request, we will let you know within 10 days of receipt of your request.

Additionally, please understand that we will continue to retain your personal information solely for our internal uses, which are consistent with the reasons for which the personal information was originally collected. We also retain personal information to assert or respond to legal rights of Freestate or other relevant third parties.

Additional Information

We may make changes to this Privacy Policy from time to time. When we review or change the policy we will update this page. You should review this page from time to time to ensure the changes are acceptable to you.
